
AutonoMe supports the local authority to identify service users most likely to benefit from AutonoMe, co-produce outcomes plans specific to the aspirations of each individual and tailors the app accordingly.
Whilst service users can access the entire library whenever they want; co-producing individual outcomes plans provides service users and their support workers with a route map to further independence in the home, motivates and focuses the mind of both service users and support workers.
Service users work through their outcomes plan and self-assess their level of independence with each task. The self-assessment function on the AutonoMe app gives service users a voice; a say in their own abilities and a direct line of communication to commissioners on a regular basis. Their self-assessment shows the amount of support they need with each skill.
